RESUMO

The Colombian mining industry has witnessed significant growth. Depending on the scale and mineral extracted, complex chemical mixtures are generated, impacting the health of occupationally exposed populations and communities near mining projects. Increasing evidence suggests that chromosomal instability (CIN) is an important link between the development of certain diseases and exposure to complex mixtures. To better understand the effects of exposure to complex mixtures we performed a biomonitoring study on 407 healthy individuals from four areas: three located in municipalities exploiting different-scale mining systems and a reference area with no mining activity. Large, medium, and small-scale mining systems were analyzed in Montelibano (Córdoba), artisanal and small-scale mining (ASGM) in Nechí (Antioquia), and a closed mining system in Aranzazu (Caldas). The reference area with no mining activity was established in Montería (Córdoba). ICP-MS measured multi-elemental exposure in hair, and CIN was evaluated using the cytokinesis-block micronucleus technique (MNBN). Exposure to mixtures of chemical elements was comparable in workers and residents of the mining areas but significantly higher compared to reference individuals. In Montelibano, increased MNBN frequencies were associated with combined exposure to Se, Hg, Mn, Pb, and Mg. This distinct pattern significantly differed from other areas. Specifically, in Nechí, Cr, Ni, Hg, Se, and Mg emerged as the primary contributors to elevated frequencies of MNBN. In contrast, a combination of Hg and Ni played a role in increasing MNBN in Aranzazu. Interestingly, Se consistently correlated with increased MNBN frequencies across all active mining areas. Chemical elements in Montelibano exhibit a broader range compared to other mining zones, reflecting the characteristics of the high-impact and large-scale mining in the area. This research provides valuable insights into the effects of exposure to chemical mixtures, underscoring the importance of employing this approach in the risk assessment of communities, especially those from residential areas.

Abstract A 33-year-old woman with a history of high blood pressure since she was 8 years old, hypothyroidism, polycystic ovary syndrome, metabolic syndrome, multiple nevi, and a maternal family history of death at age 50 due to malignant high blood pressure and heart failure. Cushing's syndrome secondary to a secretory pituitary microadenoma was diagnosed, being the cause of secondary arterial hypertension, and ruling out other causes such as renal stenosis and coarctation of the aorta. A transthoracic and transesophageal echocardiogram was performed, which detected a left atrial myxoma. Given the presence of an atrial myxoma, Cushing's syndro me and polycystic ovary syndrome, a diagnosis of Carney Complex was made due to the presence of positive Stratakis criteria. The cardiac tumor was resected, and pathology confirmed that it was an atrial myxoma. She evolved clinically stable in outpatient controls in a 6-month follow-up. Resection of the pituitary microadenoma is planned as a curative treatment for Cushing's syndrome and arterial hypertension.


Resumen Mujer de 33 años, con antecedentes de hipertensión arterial desde los 8 años, hipotiroidismo, síndrome de ovario poliquístico, síndrome metabólico, nevos múltiples y antecedente familiar materno de muerte a los 50 años por hipertensión arterial maligna e insuficiencia cardiaca. Se diagnosticó síndrome de Cushing secundario a un mi croadenoma hipofisario secretor, siendo la causa de la hipertensión arterial secundaria, y descartándose otras causas como estenosis renal y coartación de aorta. Se realizó u n ecocardiograma transtorácico y transesofágico que detectaron un mixoma auricular izquierdo. Ante la presencia de un mixoma auricular, síndrome de Cushing y síndrome de ovario poliquístico se llegó al diagnóstico de Complejo de Carney por la presencia de criterios de Stratakis positivos. Se realizó la resección del tumor cardiaco, y la anatomía patológica confirmó que se trataba de un mixoma auricular. Evolucionó clínicamente estable en controles ambulatorios en un seguimiento de 6 meses, y se planifica la resección del microadenoma hipofisario como tratamiento curativo del síndrome de Cushing y la hipertensión arterial.

Abstract Introduction: Coal worker's pneumoconiosis is caused by the inhalation of coal dust and its accumulation in the lungs after several years of exposure. Objective: To determine the prevalence of pneumoconiosis in underground mining workers exposed to coal dust in Cundinamarca, Colombia, and its association with their spirometry results and respiratory symptoms. Materials and methods: Cross-sectional study conducted on 215 workers who were administered a questionnaire on respiratory symptoms. For each worker a chest radiograph was taken according to the criteria of the International Labor Organization. Information on the workers' socio-demographic characteristics, smoking habits, and spirometry results was included. Results: The average age was 45.5 ± 9.4 years and the average length of employment was 21.7±10.0 years. Expectoration was the symptom most frequently reported (73.5%). Regarding spirometry results, a normal pattern was observed in 89.8% of the sample, while obstructive and restrictive patterns were found in 5.1% and 3.7% of the subjects, and 0.5% had a peripheral airways dysfunction pattern. The prevalence of pneumoconiosis was 42.33%. FEV1/FVC ratio (81.75 vs. 83.74, p=0.045) and FF25-75% (84.96 vs. 91.95, p<0.001) mean values were significantly lower in workers with pneumoconiosis. Conclusion: Pneumoconiosis was highly prevalent in the study population and its diagnosis was associated with spirometry results (FEV1/FVC and FEF25.75%); bearing in mind that currently there are not effective treatments for this disease, it is necessary to implement control and prevention strategies aimed at reducing occupational exposure to coal dust and, this way, prevent its occurrence.


Resumen Introducción. La neumoconiosis de los mineros de carbón resulta de la inhalación de polvo del carbón y se desarrolla después de varios años de exposición. Objetivo. Determinar la prevalencia de neumoconiosis y su relación con los resultados de espirometría y los síntomas respiratorios en trabajadores expuestos a polvo de carbón en Cundinamarca, Colombia. Materiales y métodos. Estudio de corte transversal realizado en 215 trabajadores a quienes se les aplicó un cuestionario sobre síntomas respiratorios y se les realizó radiografía de tórax según los criterios de la Organización Internacional del Trabajo. Se incluyó la información relativa a sus características sociodemográficas, hábitos de tabaquismo y resultados de espirometría. Resultados. El promedio de edad fue 45.5±9.4 años y el de experiencia laboral fue de 21.7±10.0 años. El síntoma más frecuente fue la expectoración (73.5%). En la espirometría, se observó patrón normal en el 89.8% de la población, obstructivo en el 5.1%, restrictivo en el 3.7% y de alteración de vías aéreas periféricas en el 0.5%. La prevalencia de neumoconiosis fue de 42.3%. Los valores promedio de FEV1/CVF (81.75 vs. 83.74, p=0.045) y FF25-75% (84.96 vs. 91.95, p<0.001) fueron menores significativamente en los mineros con neumoconiosis. Conclusión. La prevalencia de neumoconiosis fue muy alta en la población estudiada y su ocurrencia estuvo asociada a los resultados de la espirometría (FEV1/CFV y FEF25-75%); teniendo en cuenta que no hay tratamientos eficaces para esta enfermedad, es necesario implementar estrategias de control y prevención diseñadas para reducir la exposición laboral al polvo de carbón y, de esta forma, prevenir su aparición.

Introducción. La neumoconiosis de mineros del carbón es una enfermedad crónica e irreversible que se considera un problema de salud pública. Objetivo. Estimar la prevalencia de neumoconiosis y sus factores asociados en mineros de carbón de Boyacá, Cundinamarca y Norte de Santander. Materiales y métodos. Se hizo un estudio de corte transversal en 476 mineros. Se les hizo valoración médica, se tomaron radiografías de tórax siguiendo los criterios de la Organización Internacional del Trabajo, así como espirometrías, y se identificaron los polimorfismos de la glutatión S-transferasa y de las enzimas de reparación. Las asociaciones entre las condiciones ocupacionales y de la empresa con la neumoconiosis, se estudiaron usando modelos de regresión de Cox. Resultados. En 31 empresas se hicieron 479 monitorizaciones ambientales y se evaluaron 476 trabajadores cuyo tiempo de trabajo fluctuaba entre 10 y 57 años. La prevalencia de la neumoconiosis fue de 33,8 % (IC95% 27,0-41,3). En el modelo de regresión multivariado de Cox, con tiempo de riesgo constante para la neumoconiosis, esta se asoció significativamente con el trabajo en empresas medianas (razón de prevalencias, RP=2,00; IC95% 0,995- 2,690; p=0,052), con un nivel alto de exposición al polvo de carbón según el índice de exposición (RP=2,055; IC95% 1,043-4,048; p=0,038), y una antigüedad de 25 años o más (para 25,0 a 29,9 años: RP=2,199; IC95% 1,449-3,338; p=0,001). Conclusiones. La prevalencia de la neumoconiosis fue muy alta y se asoció a la exposición a altos niveles de polvo de carbón, a una exposición laboral mayor o igual a 25 años y al trabajo en empresas medianas.

Conclusions: The prevalence of pneumoconiosis was very high and was found to be associated with severe exposure to coal dust, work exposure for 25 years or more and working in medium-sized enterprises.

Objetivo Caracterizar la forma en que los agricultores se exponen a los plaguicidas y se presenta la intoxicación. Materiales y Métodos Estudio multi-métodos y multinivel (individuo y comunidad), que incluyó métodos etnográficos, encuesta y medición de plaguicidas en agua y muestras humanas. Resultados Se describió el proceso productivo y se conocieron los principales factores de riesgo. Los plaguicidas son considerados el mayor peligro en el trabajo y en sus viviendas. Los trabajadores tienen precarias condiciones laborales y no están protegidos por el sistema de riesgos laborales. En las muestras de agua se encontraron azinfos-metil, endosulfán, b-BHC, bromofos-metil, bromofos-etil y 2,4-DDT. En la encuesta participaron 381 trabajadores de los cuales 12,9, 68 y 5,5 % tuvieron intoxicaciones leves, moderadas y severas, respectivamente. Los casos severos tenían menores niveles de escolaridad, menor nivel de afiliación al régimen contributivo del sistema de seguridad social y un mayor número tenía enfermedad cardiovascular, diabetes, herpes u otras infecciones virales. Conclusión Existen precarias condiciones laborales que favorecen la exposición a plaguicidas caracterizadas por la exclusión del sistema de riesgos laborales, pobreza y baja educación. Es urgente la inclusión de estos trabajadores al sistema de riesgos laborales y la mejora de las condiciones de vida. De esa manera se podrán disminuir las prácticas inseguras en el manejo de plaguicidas.(AU)

Severe cases presented lower levels of education, lower levels of health care access to the contributory regimen of the Colombian social security system and higher incidence of cardiovascular disease, diabetes, herpes or other viral infections. Conclusion There are precarious working conditions that favor exposure to pesticides correlated to the exclusion of farmers from the occupational risk system, to poverty and to poor education. It is urgent to include these workers to the system of occupational risk system and to improve their living conditions, thus reducing unsafe practices when handling pesticides.(AU)

Materials and Methods A multilevel (individual and community) multi-method study, which included ethnographic and survey methods, as well as measurement of pesticides in water and human samples, was performed. Results The production process is described and the main risk factors are presented. Pesticides are considered the greatest danger at work and at their homes. Workers have poor working conditions and are not protected by the system of occupational risks. This study describes the use of pesticides mixtures and their potential association with comet assay results in 223 rice field workers in Colombia. Thirty-one pesticides were quantified in blood, serum, and urine (15 organochlorines, 10 organophosphorus, 5 carbamates, and ethylenethiourea), and the comet assay was performed. Twenty-four (77.42%) pesticides were present in the workers. The use of the maximum-likelihood factor analysis identified 8 different mixtures. Afterwards, robust regressions were used to explore associations between the factors identified and the comet assay. Two groups of mixtures--α-benzene hexachloride (α-BHC), hexachlorobenzene (HCB), and ß-BHC (ß: 1.21, 95% confidence interval [CI]: 0.33-2.10) and pirimiphos-methyl, malathion, bromophos-methyl, and bromophos-ethyl (ß: 11.97, 95% CI: 2.34-21.60)--were associated with a higher percentage of DNA damage and comet tail length, respectively. The findings suggest that exposure to pesticides varies greatly among rice field workers.

INTRODUCTION: Painters of automobiles are exposed to pure and mixed solvents that have been associated with neurological effects and carcinogenic mutations. OBJECTIVE: To characterize the health and work conditions of individuals who are occupationally exposed to organic solvents used in sheet metal and auto body shops in Bogota. MATERIALS AND METHODS: Descriptive, cross-sectional study that characterizes the health and work conditions of individuals exposed to organic solvents in sheet metal and auto body shops in Bogota. A group exposed to the solvents was compared to an unexposed group. Air concentrations of benzene, toluene and xylene (BTX) were determined, individual questionnaires were administered and phenylmercapturic, hippuric and ortho- and para-methylhippuric acids were measured in urine. The results of the measurements and the questionnaires were correlated to determine the exposure panorama. RESULTS: For the three BTX metabolites, statistically significant differences (p<0.001) were found between the population exposed to the solvents and the unexposed population. For the exposed population, positive correlations were found between toluene in air and hippuric acid in urine (rho=0.82) and between xylene in air and o-methylhippuric acid in urine (rho=0.76). Hippuric acid values exceeded permissible levels in 11 workers and p-methylhippuric acid exceeded permissible levels in 8 workers. None of the phenylmercapturic values exceeded the limit. CONCLUSION: Auto painters are exposed to high levels of organic solvents at the workplace and do not have adequate industrial health and safety conditions to perform their jobs.

Introducción. Los pintores de vehículos automotores están expuestos a solventes puros o mezclas, los cuales se han asociado con efectos neurológicos, mutagénicos y cancerígenos. Objetivo. Caracterizar las condiciones de salud y de trabajo de individuos expuestos a solventes orgánicos empleados en talleres de lámina y pintura de carros de Bogotá. Materiales y métodos. Se hizo un estudio transversal descriptivo en el que se caracterizaron las condiciones de salud y de trabajo de individuos expuestos a solventes orgánicos en talleres de lámina y pintura en Bogotá. Se comparó un grupo de trabajadores expuestos a solventes orgánicos con un grupo no expuesto. Se determinaron las concentraciones de benceno, tolueno y xileno en el aire, se hizo una encuesta individual y se midieron los ácidos fenil-mercaptúrico, hipúrico, orto y para-metilhipúrico en orina. Los resultados de las mediciones y de la encuesta se correlacionaron para establecer el panorama de exposición. Resultados. Hubo diferencias estadísticamente significativas entre la población expuesta y la no expuesta a solventes (p<0,001) en cuanto a los tres metabolitos de benceno, tolueno y xileno. Se encontraron correlaciones positivas entre el tolueno en el aire y el ácido hipúrico en la orina de los individuos expuestos (ro=0,82), y entre el xileno en el aire y el ácido o-metilhipúrico (ro=0,76). Los valores del ácido hipúrico estuvieron por encima de los límites permisibles en 11 trabajadores y, los de ácido p-metilhipúrico, en ocho de ellos. No se registraron valores de ácido fenil-mercaptúrico por fuera del límite permitido. Conclusión. Los pintores de carros están expuestos a niveles altos de solventes orgánicos en sus sitios de trabajo, y no tienen condiciones adecuadas de higiene y seguridad industrial para realizar sus labores.

BACKGROUND: Work-related diseases contribute immensely to the global burden of diseases. Better understanding of attitudes of health care workers towards occupational safety and health (OSH) is important for planning. OBJECTIVE: To assess the attitude of medical students towards OSH around the globe. METHODS: A questionnaire assessing the attitude towards OSH was administered to medical and paramedical students of 21 Medical Universities across the globe. In the current study 1895 students, aged 18-36 years, from 17 countries were included. After having performed a principal components analysis, the associations of interest between the identified components and other socio demographic characteristics were assessed by multivariate linear regression. RESULTS: Principal component analysis revealed 3 components. Students from lower and lower-middle-income countries had a more positive attitude towards OSH, but the importance of OSH was still rated higher by students from upper-income countries. Although students from Asian and African continents showed high interest for OSH, European and South-Central American students comparatively rated importance of OSH to be higher. Paramedical students had more positive attitude towards OSH than medical students. CONCLUSION: The attitude of students from lower-income and lower-middle-income towards importance of OSH is negative. This attitude could be changed by recommending modifications to OSH courses that reflect the importance of OSH. Since paramedical students showed more interest in OSH than medical students, modifications in existing health care system with major role of paramedics in OSH service delivery is recommended.

BACKGROUND: Although the number of child laborers in Latin America is generally high, data on occupational hazards and injuries is insufficient. The objective of this study was therefore to determine the lifetime prevalence of and risk factors for occupational injuries among working students (10-17 years old) in Cusco Province. METHODS: A cross-sectional study was conducted at five public night schools. 375 students (response 91.5%) completed an interview-based questionnaire on socio-demographics, work-related factors, and lifetime prevalence of occupational injuries. Multiple logistic regression analyses were performed to estimate risk factors for different types and causes of occupational injuries. RESULTS: Falls (11%), car accidents (9%) and physical violence (3%) were common causes of injuries in this population. Severe injuries (fractures, luxation or amputations) were reported by 3% of the population. A high daily income (≥20 PEN, ~15 USD) was a statistically significant predictor for injuries caused by falls [OR 2.8; 95% CI 1.2-6.5] and physical violence at work [12.1; 1.3-115.9] whereas children born in Cusco and those working in the service sector were at higher risk of injuries caused by car accidents [3.7; 1.5-9.3 and 4.2; 1.2-15.3]. CONCLUSIONS: Occupational accidents among child workers attending public night schools are common in Cusco with a lifetime prevalence of 3% for severe injuries. High income seems to convince child laborers to accept poor working conditions.

OBJECTIVES: This study was aimed at determining the living conditions, work practices, health effects and associated occupational risk factors in children working in a public market in Valencia, Venezuela. METHODOLOGY: A questionnaire was administered which included demographic and exposure variables; a descriptive analysis was then made of the data. Forty-four children were selected. RESULTS: The average workday lasted 9.2 + or - 3.2 hours/day. Children were most frequently employed in selling provisions and fruit (43.2 %); 22.7 % of them were evidently backward at school. The symptoms most reported were headache (84%), stress (59.1 %) and fatigue (58.2 %). Occupational accidents were reported by 11.4 % of the children and 2.3 % had suffered an occupational disease related to their present activity. The most frequently occurring occupational risks were discomfort caused by heat (93.2 %), noise (88.6 %) and repetitive work (84.1 %). A significant, indirect correlation was found between age and stress (p=0.04), as in publications showing that the negative impact of stress is more severe in children aged less than 10. It was determined that being a minor had greater significant association with the occupational risk of having greater exposure to organic toxic waste (p=0.017) and working under pressure (p=0.04). CONCLUSIONS: There is no systematic data collection programme for assessing child labour in Venezuela even though a variety of risk factors have been identified and perceived for children. More sensitive indicators are required for the early identification of effects and how to control exposure to the risks mentioned above so that the problem can be studied in greater depth.

Objetivo Determinar condiciones de vida, prácticas de trabajo, efectos a la salud y factores de riesgo asociados en un grupo de menores trabajadores de un mercado público en Valencia, Venezuela. Metodología Se aplicó una encuesta con variables demográficas y de exposición y se realizó análisis descriptivo de los datos, seleccionándose 44 menores. Resultados La jornada de trabajo promedio fue de 9,2 ± 3,2 horas/día. La actividad más desempeñada fue la venta de víveres/frutos (43,2 por ciento). El 22,7 por ciento de los menores presentaba retraso escolar evidente. Los síntomas más reportados fueron cefalea (84 por ciento), estrés (59,1 por ciento) y fatiga (58,2 por ciento). El 11,4 por ciento reportaron haber sufrido accidentes y 2,3 por ciento enfermedad relacionados con la actividad presente. Los factores de riesgo más reportados fueron calor (93,2 por ciento), ruido (88,6 por ciento) y repetitividad en la labor (84,1 por ciento). Se encontró una correlación significativa, inversamente proporcional, entre la edad y el estrés (p=0,04) lo cual es consistente con estudios que indican que el impacto negativo del estrés es más profundo en niños que tienen menos de 10 años de edad. Se determinó significativamente que a menor edad hay una mayor exposición a desechos tóxicos orgánicos (p=0,017) y al trabajo bajo presión (p=0,04). Conclusiones No existen procesos de recolección sistemáticos para evaluar el trabajo infantil en nuestro país aún cuando se identificaron una amplia variedad de factores de riesgo percibidas por los niños trabajadores. Para una evaluación más profunda del problema, se requieren indicadores más sensibles que permitan detectar daños tempranos y la forma de controlar la exposición a los mencionados riesgos.

Descriptive analyses and multivariate logistic regressions were conducted, with incidence and serious injuries regressed on occupational and sociodemographic variables. RESULTS: Approximately 39.6 percent of the child street-laborers surveyed reported an injury sustained while working in the streets: scratches (19.5 percent), cuts/lacerations (16.4 percent), burns (8.6 percent), car accidents (8.9 percent), sprains (4.6 percent), and amputations (0.3 percent). Working a high number of daytime hours and performing on the street predicted the greatest risk of injury, even after controlling for sociodemographic factors; specifically, each additional hour of daytime work increased the risk of injury by 1.4 percent. Child performers had three times the injury rate of children primarily selling products. Boys, older children, and children in Quito were more likely to experience moderate-to-severe injuries, than girls, younger children, and street children in the other study cities. CONCLUSIONS: Interventions are needed to address the substantial risk of injury experienced by children working on the city streets of Latin America.


OBJETIVOS: Determinar la prevalencia y la naturaleza de las lesiones laborales en niños que trabajan en las calles de cuatro grandes ciudades de América Latina e identificar los factores de predicción de esas lesiones. MÉTODOS: Estudio transversal mediante entrevistas a 584 niños de 5 a 17 años que trabajaban en las calles de Bogotá, Colombia; Lima, Perú; Quito, Ecuador; y São Paulo, Brasil. Se realizaron análisis descriptivos y regresiones logísticas multifactoriales de la incidencia y la gravedad de las lesiones con relación a las variables laborales y sociodemográficas. RESULTADOS: Aproximadamente 39,6 por ciento de los niños trabajadores de la calle encuestados informó haber sufrido lesiones mientras trabajaba: rasguños (19,5 por ciento), heridas/laceraciones (16,4 por ciento), quemaduras (8,6 por ciento), accidentes con automóviles (8,9 por ciento), esguinces (4,6 por ciento) y amputaciones (0,3 por ciento). Los factores de predicción de mayor riesgo de lesiones fueron trabajar en la calle muchas horas diurnas y realizar presentaciones callejeras, incluso después de controlar por factores sociodemográficos; específicamente, la adición de una hora de trabajo diurno aumentó el riesgo de lesión en 1,4 por ciento. Los niños que realizaban presentaciones callejeras presentaron una tasa de lesiones tres veces mayor que los niños que se dedicaban fundamentalmente a la venta de productos. Los niños varones, los de mayor edad y los de Quito presentaron mayor probabilidad de sufrir lesiones de moderadas a severas que las niñas, los más jóvenes y los que vivían en las otras ciudades estudiadas. CONCLUSIONES: Se requieren intervenciones para reducir el considerable riesgo de lesiones a que están sometidos los niños que trabajan en las calles de las ciudades de América Latina.

RESUMO

Ulnar deviation of fingers in rheumatoid arthritis is a common deformity that can markedly compromise hand function. These deformities also result in an aesthetic impairment that is often troubling to patients. A variety of surgical procedures have been recommended to correct the ulnar deviation, either at the wrist level or at the metacarpophalangeal (MCP) joints. We present a surgical technique that uses the extensor indicis proprius as a tenodesis across the dorsum of the MCP joints of the fingers. The technique can also be used in conjunction with other surgical procedures, including prosthetic replacement arthroplasties of the MCP joints. The procedure has been used in 6 patients. Ulnar deviation averaged 62 degrees preoperatively and 15 degrees postoperatively. All patients have been satisfied with the results.
